Consent Agreement

In a decision dated April 11, 2025, a panel of the NANB Complaints Committee accepted a Consent Agreement between NANB and Allan Stephen Minoza (Registration Number 031263). Mr. Minoza admitted to failing to regularly monitor and assess a patient, failing to ensure that the patient’s vitals were taken more frequently, failing to monitor and document the quantity and frequency of the patient’s diarrhea after an initial assessment, and failing to investigate further and intervene when he was told about the patient’s elevated heart rate. Mr. Minoza admitted that although he was aware of the requests from the patient’s partner for personal hygiene supplies, he failed to offer help with the patient’s care, failed to do a thorough assessment of the patient, and failed to ask the physician for an order for IV fluids. Mr. Minoza admitted that he assumed that the patient’s partner was managing the situation independently, and that his charting related to his monitoring and assessment of the patient should have been more detailed.

Mr. Minoza has voluntarily agreed to the following conditions on his registration:

  1. Remedial education related to professional communication, client assessment, and accountability;
  2. Performance evaluations from his employer(s) from the date of the Complainants Committee Decision;
  3. Not mentor or orientate any Registered Nurses; and
  4. Providing each employer and regulatory body with which he is registered with a copy of the Consent Agreement and Complaints Committee’s decision and advising NANB of any change in address or employer.
